June 21, 1940
First official communication to the War Department requesting formation
of Hospital Unit at LSU.
June 25. 1940
Letter from War Department denying request for authorization to form unit.
October 14, 1940
Second request to the War Department for authorization of unit at LSU.
October 16, 1940
Letter from War Department stating that a hospital unit at LSU had been
authorized.
October 19, 1940
Request to Commanding General 4th Army for official orders to establish
unit.
November 8, 1940
Telegram from Commanding General 4th Army stating request had been
submitted to Adjutant General.
November 27, 1940
Letter from Adjutant General stating that the request for the hospital unit
at LSU had been approved by the Secretary of War.
December 7, 1940
Letter requesting Dr. Rives be appointed Commanding Officer.
August 7, 1941
Letter to Lt Col Rives, unit commander, 64th General Hospital that he
was physically disqualified for active duty and was discharged from Reserves.
November 12, 1941
Letter from office of Surgeon General approving designation of Lt Col Ben
R. Heninger as unit director.
June 16, 17, 1942
Telegram from War Department notifying unit they would be activated during
the month of July.
